If so, you may be eligible for up to 5 rebated sessions in a calendar year with allied health providers under a Chronic Disease Management (CDM) Referral. Our dedicated CDM nurse will take you through the consultation, formulating a patient-focused plan to assist with getting you the right help from the right health professionals.
